Pershing Health System
At PHS, the primary goal of our Swing Bed Program At PHS, the primary goal of our Swing Bed Program is to help our patients return to the least restrictive environment in the shortest amount of time needed to ensure a successful transition. 

We offer complete wrap around services at discharge to help facilitate a smooth transition back to the community. Our Social Services Department will assist with discharge planning such as identifying personal support systems and referrals for the following:

➡Outpatient Occupational, Physical, and ➡Speech Therapy
➡Home health services
➡Home medical equipment 
➡Community-based services
➡Long-Term Care

 ➡️ Occupational therapy can provide a home safety assessment for our inpatient population who are returning to their home environment.
