Brookfield, MO – Pershing Health System (PHS) is proud to announce that its rural health clinics have earned two awards from their Accountable Care Organization (ACO): the Daily Huddle Reviews Award and the Comprehensive Advanced Care Planning (CACP) Referrals Award.

The Daily Huddle Reviews Award was presented to PHS for achieving the highest ranking among rural health clinics of its size in Missouri and Iowa. This recognition reflects the clinics’ commitment to improving patient care through consistent communication and coordination in daily team huddles.

The CACP Referrals Award recognizes PHS’s efforts in Comprehensive Advanced Care Planning. Similar to an advance directive, CACP helps patients communicate their healthcare wishes to providers and loved ones in the event they can no longer make decisions themselves. This process ensures that care aligns with patients’ values and goals.

These honors spotlight the dedication of the entire rural clinic team and the leadership of Amy Menconi, RHC Quality Care Specialist, whose focus on quality improvement and patient-centered initiatives was instrumental in achieving these milestones.
