Community Medical Associates is pleased to welcome Tara Swan, FNP, to its healthcare team. With nearly 20 years of experience providing primary care in rural Missouri, Tara brings a strong commitment to patient education, continuity of care, and whole-family health.
A Missouri native, Tara was born and raised in Salisbury. She earned her Bachelor of Science in Nursing from Central Missouri State University in Warrensburg (now Central Missouri University) and began her career as a registered nurse in hospital settings before transitioning to home health care. She later completed her Master of Science in Nursing at the University of Missouri–Columbia, earning board certification as a Family Nurse Practitioner.
For the past 19 years, Tara has delivered comprehensive primary care in rural communities, gaining a deep understanding of the unique health needs and expectations of patients in this setting. She enjoys caring for patients of all ages, including newborns, adolescents, women, and older adults, offering a full spectrum of care close to home.
“My favorite part of being a nurse practitioner is educating patients so they can make informed decisions about their health,” said Swan. “Not all medical information is accurate, and I strive to provide clear, honest education and support so patients feel confident in their care and can live the healthiest life possible for as long as possible.”
Tara’s approach emphasizes preventive and wellness care, management of chronic conditions, and compassionate treatment of acute illnesses—always with a focus on building trusting relationships with patients and families.

Knowles, Rural Clinic Director, said Tara’s experience and philosophy of care make her a strong addition to the clinic. “Tara understands rural healthcare and the importance of caring for the whole family,” Knowles said. “Her experience, dedication to patient education, and genuine compassion align perfectly with our mission to provide high-quality, accessible care close to home.”
Outside of the clinic, Tara enjoys spending time with her husband, twin teenage sons, their dog, and a lively group of outdoor cats, along with family and friends.
