THE CARE SUPPORT SYSTEM

Chronic Care Management Nurse – Kim Langford, LPN
Kim Langford is a key member of the Chronic Care Management team, playing a vital role in patient follow-up and ongoing care. After a patient visits the Emergency Room, discharged from the hospital, or visits a specialist Kim reaches out to schedule follow-up appointments and ensure patients are on the right path to recovery.

Kim also maintains a checklist of patients she regularly contacts to monitor their health. She checks in on important metrics such as blood pressure, blood sugar, and overall wellness, scheduling additional appointments when needed.

Serving as a direct line of communication between the patient and their provider, Kim offers one-on-one support, answers questions, and addresses concerns to help patients feel confident and cared for.

In addition to supporting patients, Kim works closely with caregivers, helping to identify any challenges they may be facing and ensuring they have the resources they need. With her extensive knowledge of local services, Kim is a vital link between patients, families, and the many community resources available to them. She helps patients and their support systems navigate the next steps in their care with clarity and confidence.

Annual Wellness Coordinator – Amy Menconi
Staying well begins with prevention, and Amy Menconi helps patients do just that. She reaches out to schedule annual wellness visits, which are key to identifying health concerns early and maintaining long-term wellness. These check-ins help patients stay ahead of potential health issues and build a strong relationship with their care team. Amy makes sure that no one falls through the cracks when it comes to preventive care.

Referral Specialist – Donna Burton
When your provider determines that you need specialized care, Donna Burton, the PHS Referral Specialist, steps in. She works directly with you to identify your preferred specialist or provides you with a list of trusted options in the area. Once you make your selection, Donna coordinates with that specialist’s office, sharing all necessary patient information to ensure a seamless transition. Donna then follows up with you to confirm your appointment details or informs you if the specialist will be contacting you directly. Her role is to take the stress out of navigating the referral process.

Prior Authorization Specialist – Cindy VanDyke
Before you undergo any procedure or test at PHS, Cindy VanDyke works behind the scenes to verify that your insurance will cover the recommended service. She communicates directly with insurance companies to obtain prior authorizations and ensures all necessary documentation is submitted in a timely manner. Cindy’s job is critical in preventing billing surprises and helping patients access the care they need without unnecessary delays. She also works closely with providers to explore alternatives if insurance requires additional steps.
